Government.
How To Order Your Credit Report By Mail

When you are ordering a copy of your credit report include the following information
in your request letter:
- Your full name, including middle name and whether you are a "Jr.," "Sr.," "III,"
etc.
- Spouse's name if you're married.
- Your social insurance number.
- Your date of birth.
- Current and former address, if you've lived at your current residence for less than
five years.
- Name of your current employer as well as any past employers.
- A daytime and evening phone number, with area codes.
Special Circumstances For an Additional Free Credit Report
You are entitled to an additional free credit report under certain special circumstances.
If you are denied approval because of negative information listed in your credit
report for any of the following:
- Credit
- Insurance
- Employment
- Job promotion
You must request the report within 60 days of being notified of the denial. When
a creditor, insurance agency or employer sends you a denial notice it must also
provide the name of the credit bureau they worked with and its contact information.
You are also entitled to a free credit report if:
- Your current employer demotes or fires you.
- Your insurance premium is increased.
- A creditor changes the terms of your credit agreement.
- You are unemployed and intend to apply for employment within the next 60 days.
- You are receiving public assistance payments.
- You believe you are the victim of credit fraud, including identity theft.
- A collection agency has told you it reported negative information about you to a
credit bureau or is planning to do so.
